Facilities have recently been issued deficiencies for failure to document an investigation and submit a report to DHH for injuries of unknown origin/abuse. For example, bruises of unknown origin to head/face should be thoroughly investigated with statements obtained from staff.

Note: During survey, if the surveyor receives from a resident an allegation of abuse, remember to submit the report/investigative outcome to DHH.

Got Pain?

A facility was issued a deficiency for failure to implement a care plan for pain. The admission pain assessment did not indicate that the resident had an issue with pain but in review of the MAR soon after admit, the resident was requiring pain meds.

Change in resident’s condition

Surveyors recently issued an IJ to a facility for failure to notify the physician and family of a change in the resident’s condition that resulted in a negative outcome for the resident.

Note: Inservice staff on proper notification of a change in condition.

Restraint Free?

Surveyors recently cited a facility for failure to assess need/appropriateness of continued use of side rails X’s 2 after a resident experienced 2 falls (with side rails in use). The corrective action of the facility was to discontinue use of side rails and the resident was placed in a low bed.

Important note: Ongoing restaint reduction/elimination assessment is key.
